At Sue Mann we understand there may be times in our life that we may need a little help. This may be to get the day started, getting out and about or help around the home – support to provide a level of comfort, confidence and sense of wellbeing to promote independence to remain living at home.
Through myhomecare, Sue Mann’s unique way of care, we listen... and then respond to your personal story and care needs. Sue Mann Nursing and Community Care (SMNCC) offers the full range of Commonwealth Home Support Programme (CHSP) funded services to support people over 65 with basic care needs.
The CHSP maximises independence through an individualised and enablement-focused range of services and supports to enhance
your quality of life in your own home.
Once assessed by the Access Point, you may be eligible for a CHSP service to support you with:
Commonwealth Home Support Programme services are subsidised by the Australian Government and require an eligibility assessment. This can be arranged by contacting the SMNCC friendly team on 1300 241 300.
You may be required to contribute towards the cost of your care. Your financial capacity will be assessed by Sue Mann’s team in accordance with our CHSP Fees Policy and you may be requested to contribute:
