myhomecare is our unique way of care. One that recognises every person is different through their experiences, family background, culture and life choices.
myhomecare helps you to build on your strengths, abilities and life passions. It’s all about maintaining your health and wellness at home. It gives you the opportunity to make informed choices about the type of care and support you, or a family member needs, and how those needs are met.
There are five steps to myhomecare. Together we create goals, develop a plan and organise the professional in-home services needed to support you living at home and improve your sense of wellbeing.
The journey to support you to remain in your local community living at home is guided by you. It starts by introducing you to your personalised myhomecare journey.
We work together to understand how involved you want to be in your care – you may want to manage the details yourself or might just be happy to steer and leave the nitty gritty to us.
Of course you change your level of involvement in directing your care at any time.
Then comes your story. You are encouraged to share your story about what brings you joy; what is most important; and how you prefer care and support to be provided.
In this process of storytelling and assessment, we get to know you a little more and begin to understand your aspirations, life’s passions, abilities and concerns.
At Sue Mann we have the local relationships and trained staff to ensure everyone has rightful access to the care and support services that suits them. Aboriginal and Torres Strait Islander elders; older Australians from culturally and linguistically diverse communities; our respected Veterans and War Widowers; and people from a lesbian, gay, bisexual, transexual and intersex community can be assured of culturally sensitive care through myhomecare .
Through listening to your story and your history, together we work out your aspirations and abilities and discuss your care needs and priorities. We establish a range of goals around the things that are important to you – the personal goals in which the care provided by us can help you achieve them. After helping to develop realistic goals and the kind of support you would like, we map out a care plan. It’s a flexible range of professional services and supports tailored to suit you.
Discussing ways to fund your care plan is another way we can help as you may be eligible for government subsidised support. Our friendly customer service team can point you in the right direction when it comes to finding the best funding solution.
With myhomecare your ongoing wellbeing is nurtured and improved. Through regular conversations and reviews, we work with you to adjust your care plan to make sure it continues to meet your changing needs and any new goals you may have.
